Injury causation is the bridge between an event and a diagnosis.

A person can be involved in a collision, fall, workplace incident, or other accident and later have a documented injury. That does not automatically establish that the event caused the condition, aggravated it, or made it clinically significant.

Causation analysis asks a narrower question: does the mechanism, timing, medical evidence, and course of treatment support a reliable connection between the event and the injury being claimed?

Depending on the case, that may require a biomechanical expert, an orthopedic surgeon, a neurologist, a radiologist, or another medical specialist. The right expert depends on whether the dispute is primarily about forces, anatomy, diagnosis, or preexisting disease.

Start With the Mechanism

The first question is usually what physically happened.

A rear-end collision, side impact, fall from height, trip, low-speed contact, lifting event, or direct blow can expose the body to very different forces.

A biomechanical expert may evaluate direction of motion, acceleration, body position, restraints, contact points, vehicle movement, or the way a person landed. Related reconstruction issues are covered on our accident reconstruction specialty page.

That analysis helps determine whether the event is mechanically consistent with the type of injury being alleged.

The expert should not begin with the diagnosis and then work backward to create a mechanism. The physical event should stand on its own.

Medical Causation Is Not the Same as Biomechanics

Biomechanics can explain how forces act on the body, but it does not replace medical diagnosis.

A biomechanical engineer may be qualified to discuss loading, motion, acceleration, or whether a mechanism is capable of stressing a particular structure.

A physician is generally better suited to diagnose a torn rotator cuff, herniated disc, concussion, fracture, nerve injury, or other medical condition and to address the patient's treatment and prognosis.

The strongest cases keep those roles separate.

The biomechanical expert explains whether the event is mechanically compatible with the claimed injury. The physician explains whether the medical evidence supports that diagnosis and whether the condition was caused or aggravated by the event. Timing Matters

Temporal proximity can support causation, but it is not enough by itself.

Symptoms that begin immediately after an event may support a relationship. Symptoms that appear weeks or months later may still be related, but the explanation usually needs to be more specific.

The expert may look at when symptoms were first reported, when treatment began, whether complaints remained consistent, and whether there were intervening events.

A gap in treatment does not automatically defeat causation. It may reflect mild symptoms, lack of access to care, or another explanation.

At the same time, a long unexplained delay can make the connection less clear.

Preexisting Conditions

Preexisting disease is one of the most common issues in injury causation.

Degenerative disc disease, arthritis, prior surgery, chronic pain, old fractures, tendon degeneration, and previous injuries may already be present before the event.

That does not mean the accident caused nothing.

The expert may need to determine whether the incident created a new injury, aggravated an existing condition, accelerated symptoms, or simply coincided with a condition that was already symptomatic.

The patient's baseline is critical. Prior records can show whether the same body part was symptomatic before the incident and whether the level of function changed afterward.

Aggravation Versus New Injury

A preexisting condition can become more symptomatic after trauma without the event creating entirely new pathology.

This distinction is common in spine and joint cases.

A patient may have longstanding degenerative findings on imaging but little or no prior pain. After an accident, the same underlying condition may become symptomatic. Related spine injury issues are covered on our spinal cord injury expert witness page.

The expert should distinguish structural change from clinical aggravation.

The absence of a new MRI finding does not necessarily mean there was no injury, but a claimed new structural lesion should be supported by objective evidence when possible.

Degenerative Spine Findings

Spine causation disputes often focus on MRI findings such as disc bulges, herniations, stenosis, arthritis, or degenerative changes.

Many of these findings can exist without symptoms and become more common with age.

The expert should not assume that an abnormal MRI proves traumatic injury.

The better analysis compares prior imaging when available, the level and pattern of symptoms, neurologic findings, mechanism, treatment course, and whether the imaging appearance is consistent with acute trauma or chronic degeneration.

In some cases, radiology and spine surgery opinions may both be useful because image interpretation and clinical causation are separate questions.

Shoulder Injuries

Rotator cuff tears are another common causation dispute.

Tears can develop from acute trauma, chronic degeneration, or a combination of both. Age, tendon quality, prior symptoms, strength, mechanism, and imaging appearance may all matter.

A fall directly onto the shoulder creates a different mechanism from a minor rear-end vehicle impact with no shoulder contact.

An orthopedic shoulder specialist may address whether the claimed tear is consistent with an acute event, whether degeneration likely predated the incident, and whether the accident aggravated an existing condition.

Knee Injuries

Knee causation can involve meniscal tears, ligament injuries, cartilage damage, fractures, and aggravation of osteoarthritis.

The mechanism matters significantly.

An ACL injury is commonly associated with particular twisting, deceleration, or contact mechanisms. Degenerative meniscal tears may occur with little or no trauma. The expert should connect the specific movement or impact to the structure alleged to have been injured.

Head Injury and Concussion

Concussion causation can be challenging because the injury may not produce abnormalities on routine CT or MRI.

A direct blow to the head is not always necessary. Rapid acceleration and deceleration of the head can occur without obvious impact.

The analysis may involve the mechanism, immediate symptoms, loss of consciousness if any, confusion, memory disturbance, headache, dizziness, and the progression of symptoms afterward.

A neurologist, neuropsychologist, or other appropriate clinician may be needed depending on the nature of the claimed impairment.

A biomechanical expert can address head movement and forces but should not substitute for medical diagnosis.

Falls

Fall cases require understanding both why the person fell and how the body contacted the ground or another object.

A forward trip may create different loading from a backward slip. A fall onto an outstretched hand may be relevant to a wrist or shoulder injury. Direct impact to the hip may matter in a fracture case. Related premises issues are covered on our slip and fall expert witness page.

Video can be particularly useful because witness descriptions of body movement are often imprecise.

The causation expert should distinguish the premises condition that caused the fall from the medical injury produced by the fall.

Motor Vehicle Collisions

Vehicle collisions are one of the most common settings for injury causation disputes.

The analysis may include impact direction, vehicle damage, change in velocity, occupant position, restraint use, seat design, airbag deployment, and post-impact movement.

Vehicle damage can provide information, but the amount of visible damage should not be treated as a direct measure of human injury.

Modern vehicles are designed to manage energy through deformation, and different crashes can produce very different occupant motion even when the vehicles appear similar afterward. Related commercial vehicle issues are covered on our truck accident expert witness page.

A proper biomechanical opinion should be based on reconstruction and occupant dynamics rather than photographs alone.

Low-Speed Collisions

Low-speed crashes frequently generate causation disputes because the vehicle damage may be limited while the plaintiff reports substantial symptoms.

The relevant question is not whether the collision looked minor.

The expert may evaluate the estimated change in vehicle velocity, acceleration, occupant movement, body position, prior medical condition, and whether the claimed injury is mechanically plausible.

The analysis should avoid simplistic thresholds.

There is no single speed below which injury is impossible, and there is no single amount of vehicle damage that proves injury occurred.

The opinion should be specific to the event and the person.

Workplace Injuries

Workplace causation cases may involve lifting, repetitive motion, falls, equipment impact, vibration, or sudden exertion.

The expert may need to determine whether the physical demands of the task were capable of producing the claimed injury or aggravating a preexisting condition.

Ergonomics and occupational medicine may become relevant when the allegation involves cumulative trauma rather than a single event. Related human factors analysis can also matter when task design and worker interaction are disputed.

The analysis should distinguish acute injury from disease that developed gradually over time.

Repetitive Stress and Cumulative Trauma

Some injuries are alleged to result from repeated activity rather than one identifiable accident.

Carpal tunnel syndrome, tendinopathy, back pain, and other musculoskeletal conditions may be attributed to work tasks or repeated exposure.

These cases require a different causation model.

The expert may evaluate frequency, duration, force, posture, repetition, recovery time, and known medical risk factors.

A temporal relationship between work and symptoms is relevant, but the expert should explain why the exposure was sufficient to cause or materially contribute to the condition.

Alternative Causes

A good causation opinion should consider other plausible explanations.

The patient may have experienced a prior injury, subsequent accident, underlying disease, sports activity, surgery, infection, metabolic condition, or another event capable of producing similar symptoms.

The presence of an alternative cause does not automatically eliminate the incident being litigated.

The expert should evaluate relative timing, medical evidence, and plausibility rather than simply list every theoretical alternative.

An opinion is more persuasive when it explains why one mechanism fits the evidence better than the others.

Objective Findings and Subjective Symptoms

Pain is real even when it cannot be measured directly.

At the same time, injury causation often relies on a combination of subjective symptoms and objective medical findings.

Imaging, physical examination, weakness, sensory loss, range of motion, electrodiagnostic testing, surgical findings, and other objective evidence may help support the diagnosis.

Some legitimate injuries produce limited objective findings, particularly early in their course.

The expert should evaluate the evidence appropriate to the claimed condition rather than assume that every injury must appear clearly on imaging.

Treatment Consistency

The treatment course can help test causation.

A patient who reports shoulder pain immediately after an accident, receives repeated treatment for the same shoulder, undergoes imaging showing a relevant abnormality, and ultimately requires surgery presents a different causation picture from someone whose complaints shift among unrelated body parts over time.

That does not mean consistent treatment proves the event caused the injury.

It means the course should make medical sense.

Experts may examine whether the diagnosis, symptoms, treatment, and objective findings remain coherent from the incident through later care.

Prior Medical Records

Prior records can be some of the most important evidence in a causation case.

They establish whether the patient had previous complaints involving the same body part, whether imaging abnormalities were already present, and what the person's functional baseline looked like.

The absence of prior symptoms can also matter.

A patient may have degenerative findings before an accident but no documented complaints or treatment. The expert must decide whether the incident reasonably explains the transition from asymptomatic disease to symptomatic impairment.

Prior records should therefore be used to establish baseline, not simply to identify any abnormality that existed before the event.

Subsequent Accidents

A later event can complicate causation significantly.

The patient may sustain a second collision, fall, sports injury, or other trauma while still being treated for the original incident.

The expert may need to allocate which symptoms or structural changes are attributable to each event.

That can be difficult when the same body part is injured twice.

Records immediately before and after the second event often become critical because they show whether the patient's condition had been improving, stable, or already severe before the new trauma occurred.

Surgery After an Accident

The fact that a patient eventually undergoes surgery does not establish that the accident made the surgery necessary.

The expert should determine what condition the procedure treated and whether that condition was caused or aggravated by the event.

A patient may have had severe preexisting arthritis that was already approaching surgical treatment. Another patient may have been functioning well until an accident produced a new structural injury that directly led to surgery. The surgical indication and the patient's pre-incident history matter more than the existence of the procedure alone.

When Biomechanics Helps Most

Biomechanics is most valuable when the physical mechanism itself is disputed.

It can help evaluate whether a body moved in the manner alleged, whether a particular structure was loaded, or whether the forces from an event are consistent with the claimed injury mechanism.

It is less useful when the main dispute is purely medical, such as whether a documented disease process was caused by an infection or whether cancer progression resulted from delayed diagnosis. Related delayed diagnosis issues are covered on our delayed diagnosis expert witness page.

The expert discipline should follow the actual causation question rather than using a biomechanical expert simply because an accident occurred.

When a Physician Should Lead

Some injury cases can be answered primarily through clinical medicine.

If the event clearly involved substantial trauma and the dispute concerns whether a particular diagnosis is related to it, an orthopedic surgeon, neurologist, or other treating specialty may be the central causation expert.

The physician can integrate symptoms, examination, imaging, treatment, surgery, and prognosis.

Biomechanics can still be supportive, but it should not replace the specialty that actually diagnoses and treats the condition.

Avoiding Overstatement

Causation testimony becomes vulnerable when experts claim more certainty than the evidence supports.

The medical record may support that an event probably caused an injury. It may support aggravation but not a wholly new condition. It may also be impossible to distinguish among several plausible causes with confidence.

A strong expert identifies those limits.

Causation opinions are more defensible when they explain what the evidence supports and what it does not rather than treating every post-accident diagnosis as attributable to the event.

Building the Causation Timeline

The cleanest causation analysis usually follows a chronological sequence.

What was the patient's condition before the event? What physically happened? What symptoms appeared immediately afterward? What did the first examination show? What changed on imaging or testing? How did treatment progress? Were there intervening events?

That timeline makes it easier to identify whether the claimed relationship is medically coherent.

It also exposes gaps that deserve explanation rather than allowing the final diagnosis to dominate the analysis.

Choosing an Injury Causation Expert

There is no single credential for injury causation.

A biomechanical engineer may be appropriate when the case turns on forces and mechanism. An orthopedic surgeon may be the right expert for a joint, fracture, or spine injury. A neurologist may be necessary for brain, nerve, or neurologic claims. Radiology may be relevant when the age or character of imaging findings is disputed. Complex cases may require more than one expert because mechanical plausibility and medical causation are different opinions.
